Folliculitis keloidalis, also known as folliculitis keloidalis nuchae or nodular scarring of the neck, is a special form of folliculitis that can lead to scarring alopecia. This condition typically affects men of African or Caribbean descent and is characterized by inflamed and scarred hair follicles in the neck area.

In contrast to normal folliculitis, where the inflammation of the hair follicles is usually caused by infection or irritation, in keloidal folliculitis a chronic inflammation develops that leads to scarring of the affected area. The exact cause is not fully understood, but repeated irritation from shaving, ingrown hairs or other mechanical influences may play a role.

Symptoms of keloid folliculitis include red, itchy bumps or nodules on the back of the neck that can develop into larger, painful nodules over time. These nodules can develop into thick, fibrotic scars that can interfere with hair growth and lead to permanent hair loss.

Treatment of keloid folliculitis can be challenging and often involves a combination of topical or oral antibiotics, steroids and other anti-inflammatory medications to reduce inflammation. In some cases, surgical removal of the affected tissue or laser therapy may be required to reduce scarring and promote hair growth. Prevention by avoiding irritants in the neck area is also important to minimize the risk of keloid folliculitis.
